i forget my PUK Code and Pin Code

this is antony sunny. i m ur customer since 1 year from mumbai. by mistake i locked my sim and i forget my PUK Code and Pin Code . I really don`t know wat 2 do to get the pin code.if u don`t mind can u send me my PUK Code and Pin Code by my Email id .
